Before you begin — a few notes
  • Adult students — Student Grade and all Guardian fields (Step 3) are not required. Please leave them blank.
  • Students under 18 — please complete all Guardian fields in Step 3 including emergency contact details.
  • Furigana fields — please enter the reading of the student's name in katakana (カタカナ).
  • Medical fields — if there are no allergies or conditions, please write "None".
  • All information is kept strictly confidential and used only for enrollment purposes.

  • 18歳未満の生徒の方は、ステップ3の保護者情報および緊急連絡先を必ずご記入ください。
  • フリガナ欄は、生徒のお名前をカタカナでご記入ください。
  • アレルギー・病歴欄は、該当がない場合は「なし」とご記入ください。
